Description
$10,000 sign on incentive with Service Agreement for full-time employment - $5,000 paid after 90 days of employment in three monthly installments and $5,000 paid at the completion of the first year as a second year incentive in three monthly installments.
Wage rate is determined by education (BSN) and number of years as a licensed RN
Essential Functions
Responsible for the admission of all hospice eligible patients to the services of Columbus Hospice. Coordinate with area hospital case managers and other health care providers as appropriate.

Challenges in Position
Work hours may extend moderately beyond schedule to complete the admission process.
Flexible work schedule that does not include nights or weekends, but may work day hours on a rotational basis to cover holidays that fall on weekdays.
Prioritizing and coordinating patient and family needs.
Frequent interruptions.
Dealing with grief, high anxiety, and emotions of death and dying.
Frequently engaged in difficult conversations with patients and families.

Act on physician orders as directed.
Perform assessments to assist in the determination of hospice eligibility.
Coordinate services to include medical equipment, medications, treatments, and care team.
In Collaboration with Management
Anything not currently defined by Georgia and Alabama State Nurse Practice Act, policy and procedures, state license regulations, or appropriate physician orders.

Education and Experience
Associate's degree or equivalent from two-year College or technical school; and one year related experience.
Certificates, Licenses, Registrations, Vaccines
Current Georgia and Alabama nursing license required
Current CPR Certification required
Certification in hospice and palliative care preferred, but not required for application; desirable to obtain within two years of start of position.
Valid state drivers license with a good driving record required
Automobile insurance coverage per policy requirement required
Influenza vaccine during influenza season required (Oct. - March)
COVID-19 vaccine (when required by policy)
Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent. Ability to calculate figures and amounts such as medication doses.
Reasoning Ability
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to appropriately present information to families, patients, and peers.

Position Requirements (not all-inclusive)
Accept referral information from physicians, patients, families, physician offices, discharge planners, and other sources.
Effectively deliver hospice information to patients and families as requested.
Complete patient assessments.
Assist the care team in determining patients eligibility to receive hospice benefits.
Secure admission paperwork, and physician orders.
Complete internal referrals for needed services.
Secure medical equipment, supplies, and medications, as ordered.
Consult with primary physician for initial orders and symptom management orders.
Initiate the plan of care based on patient and family needs to include physical, social, and spiritual.
Communicate patient information to Team Manager for primary nurse assignment and to the on call nurse.
Serve as a resource of alternative services available in the community for the patient.
Develop a hospice presence in area hospitals and oncology units by making rounds, attending discharge-planning meetings, and collaborating with case managers.
Participate in staff and team meetings as required.
Obtain staff development education to equal 15 hours per evaluation year as per Columbus Hospice policy.
Participate in performance improvement activities.
Participate in Joint Commission accreditation processes of Columbus Hospice, Inc. and surveys by other regulatory and accreditation agencies.
Attend mandatory in-service education and staff meetings.
Attend a minimum of one memorial service per evaluation year.
Promote and integrate the mission and values of Columbus Hospice and model the behavior and standards set forth by the mission statement.
Take all reasonable precautions to maintain privacy and non-disclosure of any confidential information about patients, other staff members, and the general operations of Columbus Hospice.
Accurately enter information into timekeeping system in order to document arrival, departures, and mileage.
Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.
Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job.
Utilize provided technology to enter documentation at point of care in near real time.
When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor.
Ensure job responsibilities are met, including attendance requirements and prompt arrival at work.
Assist with special projects, as requested.
