Position Goal
The Infectious Disease Nurse Navigator is an experienced registered nurse who is accountable for the effective clinical care coordination of patients diagnosed with wound infections. She coordinates the care of patients receiving ongoing community-based and/or home health treatment for infectious conditions. She works closely with Infectious Disease physicians and infusion providers to manage the patients’ transition from acute to community-based and/or home care settings and coordinate initiation and ongoing management of intravenous anti-infective therapies.
Position Responsibilities:

Functions as the liaison for physicians, managed care companies, and other customers.

Collaborates with physicians to ensure quality outcomes.

Provide a consistent point of contact for the patient and their family.

Provide education, treatment options, and importance of treatment adherence.

Coordinates efforts for treatment and promotes quality improvement and educational efforts.

Monitor patients’ progress and advocate for necessary services and follow-up care.

Assists with referrals/support for follow-up care.

Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.

Assists medical provider with processing of diagnostic results bringing significant values to the provider’s attention. Exhibits knowledge of tests and procedures and their significance as related to patient care.

Recognizes the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients. Uses knowledge of medications in instructing patients about medication therapy.

Establishes working relationships teaching protocols for patient education, family counseling, and general information.

Performs other duties as assigned.
Experience: A minimum of 3 (three) years acute care experience preferred.
Education: Associate of Science in nursing from an accredited institution required. Bachelor of Science or higher degree in Nursing from an accredited institution strongly preferred.
Special Qualifications:
Required:Â

Current AL RN licensure, or multi-state licensure, in good standing.

Must understand process improvement, team dynamics, medical model and financial structure of health care.

In-depth knowledge of IV, central venous catheter (CVC), and peripherally inserted venous catheter (PICC) troubleshooting and complications required.

Working knowledge of pharmacology of common anti-infective therapies required.

Case management skills preferred.

Proficiency in management of intravenous therapy and infusions.

Proficiency in telephone triage.

Excellent oral and written communication skills.

Strong organizational, problem-solving, decision-making and critical thinking skills.

Basic computer skills.
