Responsibilities
- Manages patients enrolled in remote monitoring (RPM) services including blood pressure and weight modalities
- Schedules patient appointments using the Electronic Health Record
- Enrolls patients in the RPM program and educates on proper use of clinic-provided equipment
- Communicates with enrolled patients via phone to meet Centers for Medicare & Medicaid Services (CMS) billable criteria for RPM services
- Conducts follow up calls and manage documentation of care with enrolled patients per organization policies and guidelines
- Collaborates with provider teams to manage care of enrolled patients
- Manages equipment inventory for RPM services
- Coordinates with team members and leader(s) to ensure appropriate inventory of equipment; identifies equipment needs in a timely manner
- Coordinates with leader(s) and the billing department to publish charges for unreturned equipment
- Ensures documentation needs are met to effectively bill for RPM services
- Coordinates with Billing department to maintain CMS criteria is being met and practices are current
- Completes documentation in the EHR in a timely manner to ensure claims are accessible
- Other duties as assigned
Education and Qualifications
- Certified Medical Assistant (CMA)
- Maintains Basic Life Support (BLS)
Job knowledge and skills
- Cardiology experience of at least 1 year
- Moderate computer skills to include, but not limited to, use of Electronic Health Records system, use of Excel Spreadsheets, and web-based portal(s)
- Communicating with compassion, both within the clinic and with patients/families
- Critical thinking and prioritization of tasks
- Works well in a team environment
