Job Summary
Under the general supervision of the Case Management Director, the supervisor of case management serves as a resource for the case management team and other departments. Responsible for providing direct supervision of the case managers.
Essential Functions
- Conducts assessments and document conditions, observations, and actions.
- Supports case management coordination of patient transfers to acute care facilities to ensure compliance with hospital policies/procedures and regulatory requirements.
- Assist with managing the department in a cost-effective manner by utilizing current materials management techniques, maximizing human resources and continuously reevaluating the manner in which services are delivered.
- Coordinates assignment of case management staff to meet required services in each area.
- Supports case management coordination of patient transfers to ensure compliance with hospital policies/procedures and regulatory requirements.
- Directs, coordinates, and provides case management to patients.
- Facilitate patient and/or family counseling sessions; establish goals and care plans.
- Participate as a contributing member, in the discharge planning process.
- Serve as a liaison between hospital departments, to include discharge planning and social service.
- Other duties as assigned.

Qualifications
Education
- Associate Degree in Nursing (ADN) Required
- Bachelor's degree Preferred
Licenses/Certifications
- Professional Registered Nursing licensure in state of GA.
- Basic Life Support (BLS)
Experience
- Three (3) years of acute care experience
- Previous Case Management experience
