Qualifications: Bachelor’s degree in social work, psychology, criminal justice, sociology or related social services field plus two years’ experience with children and families; or Master’s in social services field and one year of related experience. Foster care/mental health experience preferred. Organizational Relationships: Reports to TFC Supervisor; may report to or partner with TFC Director or other supervisors; works as part of a TFC team and with resource development staff. Description of Key Responsibilities: Provides comprehensive case coordination for children in therapeutic foster homes; visits children and foster parents regularly; monitors Master Service Plans and children’s health/educational/social needs; coordinates mental health, medical and educational services; leads treatment team meetings and plan reviews; provides technical assistance and support to foster parents; supports birth families and documents contacts; acts as liaison to community resources and attends court/panel/school/clinical reviews; provides crisis intervention and rotates 24/7 on-call; maintains documentation and records (progress notes, incident reports, GA Score data); processes client and foster parent data for financial purposes; completes respite and placement change reporting; performs other duties as assigned.
