Qualifications: Bachelor’s in social work/psychology/criminal justice/sociology or related +2 years’ social services experience with children/families; or Master’s +1 year. Foster care and/or mental health experience preferred. Reports to TFC Supervisor; works within a team approach. Key responsibilities: Provide comprehensive case coordination for children in therapeutic foster homes; frequent child/foster parent visits (at least biweekly/weekly based on need); implement and monitor Master Service Plans; ensure health, educational, social, and emotional needs are met; coordinate services and educational supports (including ILP for teens); support and train foster parents; provide respite coordination and transportation; support birth families and document contacts; act as community liaison and attend courts/panels/meetings; provide crisis intervention and 24/7 rotating on-call; maintain documentation (contact notes, monthly reports, incidents within 24 hours, GA Score data); process financial and placement change data; perform other duties as assigned.
