Qualifications: Bachelor’s in social services plus two years’ experience, or Master’s and one year; foster care system and/or mental health experience preferred. Organizational: Reports to TFC Supervisor; works within a team; partners with resource development staff. Key Responsibilities: comprehensive case coordination; frequent visits with children and foster parents; develop and review Master Service Plans; coordinate mental health/medical/educational services and ILP; support and train foster parents, arrange respite, transport as needed; support birth families; liaison/advocate with community; provide crisis intervention and on-call support; documentation and reporting (contact notes, monthly summaries, incident reports, GA Score); process financial/placement change data; complete funding applications; other duties as assigned.
