Qualifications: Bachelor’s in social work, Psychology, Criminal Justice, Sociology or other social services field plus two years’ social services experience with children/families; or master’s in social services field and one year of related experience. Foster care/mental health experience preferred. Organizational relationships: reports to TFC Supervisor; may partner with TFC Director/other supervisors; team approach with TFC staff; partner with resource development. Key responsibilities: comprehensive case coordination for children in therapeutic foster homes; frequent child/foster parent visits; implement/monitor Master Service Plans; ensure health, educational, social and emotional needs; coordinate mental health/medical services; education advocacy and ILP activities; provide support and training to foster parents; arrange respite and transport; support birth families and document contacts; serve as community liaison and attend court/school/clinical meetings; provide crisis intervention and rotate 24/7 on-call; complete documentation (contacts within 72 hours, monthly reports by 10th, incident reports within 24 hours, GA Score data); process financial data (respite reports, resident change sheets, funding applications); perform other duties as assigned.
