Qualifications: Bachelor’s degree in social work, psychology, criminal justice, sociology or related field plus two years of social services experience with children and families; or master’s degree in social services and one year of related experience. Foster care and/or mental health experience preferred. Organizational Relationships: Reports to the TFC Supervisor; may partner with TFC Director/other supervisors; works in a team with TFC staff and resource development staff. Key Responsibilities: Provides comprehensive case coordination for children in therapeutic foster homes; conducts regular visits with children and foster parents; ensures implementation and monitoring of Master Service Plans; coordinates mental health, medical, educational, and ILP services; provides technical assistance and training support to foster parents, arranges respite, and transports children as needed; supports birth family engagement and documents contacts; acts as liaison and advocate with community resources, courts, schools, and clinical providers; provides crisis intervention and participates in 24/7 on-call rotation; maintains documentation/records, monthly reports, incident reports, and GA Score data; processes financial-related data, respite reports, resident change sheets, and funding applications (RBWO/MAAC); performs other duties as assigned.
